I hereby make an application for training in JiuJitsu, MMA, summer camps or the fitness classes at the Creighton JiuJitsu, Inc. Academy, and upon acceptance, I sincerely pledge to obey all the rules and regulations of this class. The rules and regulations are set up for the purpose of keeping order and for the protection of the students from injury. I recognize that a potential risk of injury is involved in the study of JiuJitsu, MMA or any of the fitness classes and therefore requires my strict adherence of these rules and the Instructor’s.
Any person who fails to conduct themselves in a proper manner, or who engages in any type of misconduct or criminal activities, or who earns disrespect through their attitude or actions will be required to resign from the JiuJitsu, MMA or fitness classes. The Instructor will be the judge as to who is in violation of this code of conduct. Every student is required to conduct himself or herself in a proper and honorable manner. Use of illegal substances, excessive use of alcoholic beverages, and/or poor representation of the martial arts, will also be considered reasons for termination.
In consideration of my acceptance to this class, I realize that this type of training requires vigorous physical and mental activities, and attest that there are no medical conditions that will affect my participation. I hereby waive any and all claims, actions, causes of action, demands, costs, loss of service claims, expenses, and loss of compensation claims, whatsoever, which the undersigned may hereafter accrue on account of, or in any way growing out of any and all known and unknown, foreseen and unforeseeable bodily injuries, property damage, and consequences thereof resulting from any instruction, training, or any athletic contest or events associated with this class, including travel to and from class, or contest or activities, and hereby assume all risks incident thereto. Furthermore, I hereby release Creighton JiuJitsu, Inc. Academy, Paul Creighton, the course instructor; any assistant instructors in the course or summer camp instructors; and any guest instructors or clinicians from any and all liability of whatsoever kind or nature consistent with, but not limited to, the general waiver executed in this agreement.
I understand and agree that I shall assume all responsibilities of my actions should I use the training received from JiuJitsu, MMA, fitness classes, the instructors, assistant instructors, and seminar representatives to cause injuries or damage to any person or property. Having fully read and understood the above, I agree to all aforementioned terms and conditions and hereby apply for membership in JiuJitsu, MMA or the fitness classes at the Creighton JiuJitsu, Inc. Academy.
